2022 Immunizations

Please be sure to submit paper copies of all immunizations.  Remember to sign them!!!

If your child does NOT have a birthday this summer (in which they will be receiving immunizations) please go ahead and turn in a SIGNED PAPER copy of your child’s updated immunizations before the end of this school year (June 17th).  This will really help Marilu.

If your child is either NEW to the school or has a birthday (in which they will be receiving immunizations this summer) please be sure to submit their new, completed and SIGNED immunization records to Marilu by August 15.  Please note: photos of your forms will not work.  Please mail a hard/paper copy to the school:  2204 NE Birch Street, Camas, WA 98607. Please check here to verify your child is up to date. (Returning students who have not had any updates do not need to resubmit.) State law says no student may attend a class until immunization records are on file.
All students are required to have medically verified immunization records on day one for entry into any public or private school and child care center.
Medically verified record includes one or more of the following:
  • A physical copy of the CIS form with a healthcare provider signature
  • A CIS printed from MyIR (for parents to log-in)

 

School and Child Care Exemptions from Immunization Requirements

Washington State Law allows parents or guardians to exempt their child from the school or childcare immunization requirements. Exemptions may be claimed for personal/philosophical, religious or medical reasons. Measles, mumps, and rubella may not be exempted for personal/philosophical reasons. To request an exemption a completed Certificate of Exemption must be submitted to the school or child care center.

 

Excluding Children Out-of-Compliance with Immunization Requirements

By law, RCW 28A.210.120, it is the duty of the administrator of every public and private school and day care center to prohibit the further presence at the school or day care center for any and all purposes of each child for whom proof of immunization, certification of exemption, or proof of compliance with an approved schedule of immunization has not been provided in accordance with RCW 28A.210.080 and to continue to prohibit the child’s presence until such proof of immunization, certification of exemption, or approved schedule has been provided. Students lacking immunization records who are covered under the federal McKinney-Vento Act may not be excluded. The exclusion of a child from a school shall be accomplished in accordance with rules of the Office of the Superintendent of Public Instruction, in consultation with the State Board of Health WAC 392-380-050 (Public schools) WAC 180-38-050 (Private schools).
